registering my dog

I just got my first papered dog and i need to know how to fill the registration forms out. Can some one explain the registered name and call name.

The registration name is his "official" or "professional" name. His call name is just the regular name by which you will always call him.

For example, 1 of my dog's registered name is "S.C.'s Shakin' It Up"

his call name is "Shaker"
